Initial Comments:
Based on the findings of an onsite unannounced complaint investigation survey completed January 12, 2024, Moravia Health was found not to be in compliance with the requirements of Pa. Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart A., Chapter 51.

51.3 Notification

(f) If a health care facility is
aware of a situation or the occurrence
of an event at the facility which
could seriously compromise quality
assurance or patient safety, the
facility shall immediately notify the
Department in writing.
The notification shall include
sufficient detail and information to
alert the Department as to the reason
for its occurrence and the steps which
the health care facility shall take to
rectify the situation.

Observations:

Based on review of event reporting system ERS, consumer files (CF), and interview with agency staff (EMP) the failed to notify the Department in writing of a recurrent situation that seriously compromised patient safety for one (1 ) of three (3) CF reviewed. CF #1

Findings include:

Review of Department's event reporting system completed 1/9/2024 at approximately 1:00PM revealed no report made by the agency that could seriously compromise patient safety during November or December 2023.

Review of consumer files completed January 12, 2024 between approximately 9:00AM and 10:30 AM revealed the following:

CF #1, start of care (SOC): 7/18/23, contained documentation showing a service agreement for care to be provided 11/4/23, 11/5/23, 11/18/23, 11/19/23, 12/2/23, 12/3/23 &12/16/23, 12/17/23. No doucmentation of care being provided as scheduled.

An interview completed January 12, 2024 with agency staff confirmed the above findings.

Based on the findings of an unannounced complaint investigation survey, conducted on January 12, 2024, Moravia Health was found to be in compliance with the requirements of 28 Pa. Code, Health Facilities, Part IV, Chapter 611, Subpart H. Home Care Agencies and Home Care Registries.
